Episcopal Health Services has reached a Settlement in a class action lawsuit involving a Data Incident that occurred between August 28, 2018 and October 5, 2018.
A Settlement Agreement has been reached in a class action lawsuit (the “Lawsuit”) alleging that, between August 28, 2018 and October 5, 2018, one or more email accounts at Episcopal Health Services, Inc. (“EHS”) containing information pertaining to Class Members was accessed by unknown, unauthorized user(s) (the “Data Incident”). The information exposed included names, addresses, dates of birth, medical information, and/or Social Security numbers of some Class Members.
As part of this Settlement, EHS has agreed to reimburse eligible Class Members for certain types of claims, provide Identity Monitoring Services, and undertake certain preventative cyber security measures. You must submit a Claim using a Claim Form in order to receive any recovery under the Settlement Agreement.

You may enroll in Identity Monitoring Services through Kroll for 24 months by following the instructions you received in the mailed Notice. You must enroll and activate this coverage by April 19, 2021.
Ask to be Excluded
Exclude yourself from this Lawsuit, meaning you will receive no benefits from this Settlement and will not be bound by the release of claims.
Object to the terms of the Settlement.
You will not receive compensation or additional Identity Monitoring Services and you will give up your rights to sue EHS about issues related to the Data Incident.
